We accept the following Medicaid Insurances: Passport, Humana CareSource, Wellcare, Aetna, Anthem, and Kentucky Medicaid.
We also accept select private insurance as well as self-pay — if you’re not sure if we accept your insurance, give us a call or use the contact form today.
We have employees in each office who can assist with benefits acquisition for free – they will help assess which insurance might be best for you, and walk you through the application process.
You could very well leave with insurance that same day – call now to inquire.
Appointments can be made by phone, the appointment tabs and forms on this site, as well as by walking-in to our office at 5115 S. 3rd street in Louisville, KY.

All you need to bring is a valid ID and insurance card; we will also be asking you for information such as your name, SSN, address, and phone number. That’s it!
When you come in you will take a urine drug screen and if you are clear of all substances, you may get a prescription that day — every case is taken on an individual basis and with the patient’s safety in mind.
Our goal is to help get you off of drugs, not dependent on other substances long-term — our psychiatrists & counselors will assess you and help make the best decision.
As long as you trust this person to be in sessions with you, we support whatever makes you feel safest and promotes your best chance at a successful outcome. Keep in mind that at some point individual, one-on-one therapy may be suggested to reach your greatest potential.
Case Management is a program funded by Medicaid to support clients with their high-level needs and also their life’s goals. Common needs and goals include: housing, transportation, food/SNAP benefits, SSDI filing/appeals, legal assistance and referrals, educational goals, employment, finanacial assistance, childcare, and much more.
We have 3 Louisville Locations, you can come to any you choose, unless you need Medication Management, IOP, and/or MAT – then you will need to come to the location at 5115 S. 3rd street, Louisville, 40214.
